  If you are a Caregiver to someone currently undergoing cancer treatment, consider joining us for our upcoming Coping With Caregiving Conference Calls. These toll-free and complimentary calls are hosted by caregivers for caregivers and focus on a different topic every month to help you deal with the stress of a cancer diagnosis.

Campsite Information Form

The top 20 fundraising teams will choose first, then all others will be assigned according to when your form was turned in, if you're fundraising onsite, and the number of people staying at your site. Car Raffle Request Form
  Use this form to request the raffle car to be at your team fundraiser. You will be responsible for picking up and dropping off the car from the American Cancer Society office at 4416 Expressway Dr., Virginia Beach, VA 23452 and for refilling the fuel to the same level.

  The 2010 Incentive Prizes are available and one of these great items can be yours! All you have to do is fundraise.
Luminaria Form
  Sell Luminaria bags "In Honor Of" or "In Memory Of" a loved one. The recommended donation per bag is $10. You can pick up bags at Team Meetings. Those you sel them to are encouraged to decorate them with artwork, photos, poems, etc. to make them unique and special.
March Madness Raffle Car Challenge
  Sell car raffle tickets from March 8 through April 8 and you will be in the running for some wonderful prizes. The top team and individual across our area will win. See the flier for additional details.

Miss Relay Flier
  Sign up your "prettiest" team member for this hilarious competition. The person with the most money in their purse at the end of Relay night will be crowned Miss Relay. This year we will also award a prize for the most talented "lady".
Night of Event Need to Knows
  This document lists everything you'll need to know about the day of the event from parking to porta potties. Please remember that pets, tobacco, and alcohol are not allowed at any Relay event.
Norfolk Admirals Relay Night Sales Form
  Sell vouchers to the March 6 Admirals hockey game and your team will receive credit for $2.00 of every ticket that you sell. The ttop 2 teams who sell the most tickets will get to play broomball during the 2nd Intermission.
Pet Luminaria Form
  Our beloved pets can also fall victom to cancer, so why shouldn't we honor them for their fight? You can sell Pet Luminaria bags "In Honor Of" or "In Memory Of" a furry, scaly, or feathered friend. Pet bags are smaller than their human counterparts', and have a recommended donation amount of $5.00. Bags can be picked up at Team Meetings. We encourage those who purchase the bags to decorate them with photos, poems, artwork, etc. to make them unique and special.

  Before you can check out tickets for the car raffle, you must read and agree to the rules listed on this sheet. You can bring this form with you to the Society office or to the Team Meetings to expedite the process.
Registration Form
  Download this form and submit it to any team meeting or to the American Cancer Society office with your $10.00 registration fee. This form works for participants registering to teams as well as Survivors registering for the Survivor Reception.
Relay Bucks Activities to Earn Points
  Make sure you earn all of your deserved Relay Bucks for your chance to win one of three fabulous prizes: a private porta potty, a VIP Parking Space, or $100 towards your team total.
Road to Recovery Race Rules & Entry Form
  Sometimes the biggest obstacle to a cancer patient's recovery is getting to treatment. At the Society, we have a program called Road to Recovery, where volunteers drive cancer patients to and from their treatments. To promote the program & have fun while doing it, your team is encouraged to create a mode of transportation to carry your patient to treatment (use our military theme & be creative). Awards will go to the best looking vehicle as well as the fastest & most careful driver.

  What an easy way to raise $100 for your team. Sell a track sign to local businesses or organizations for $100. Their name and information will be displayed around the track for 15,000 people to see for 15 hours. Its a great way for them to advertise and show their support for their community as well as a great way to fundraise. All $100 goes towards your team fundraising goal.
